This is a condition where the sciatic nerve gets compressed by the inflamed piriformis muscle. Pain in the buttocks which may radiate down through the back of the thigh is the major symptom. Numbness in the buttocks can also be seen in certain cases. Pain and the associated complaints aggravate by sitting, Usually the reason for this issue is continuous stress or trauma on the piriformis muscle. This could be due to long term sitting on hard surface.

This is also very common in people who place their wallet in their back pockets. Ayurveda offers good prognosis in treating such conditions. It can be related to a condition where Vata is aggravated in Piriformis muscles due to stress and obstruction in its activity at the grudhrasi nadi. This leads to pain in and around the area of inflammation and on the course of the sciatic nerve. This is managed with therapies and internal medications. The first approach is to reduce the inflammation and then followed by strengthening of the muscle to prevent further relapse. With appropriate internal medicines, massage of the affected area with selected medicated oils, massage with herbal powder bag (podi kizhi) heated in the appropriate warm medicated oil, massage with herbal leaf bag (ela kizhi) dipped in warm medicated oil, pouring medicated oil all over the body (pizhichil) , cooked medicated rice bag massage (njavara kizhi), keeping medicated oil pool on the affected area (kati vasti), application of the herbal paste on low back, bandage of the low back with medicated oil. Panchakarma therapies like enema with herbal decoction and/or medicated oil are the procedures usually performed in the management of these condition. Integrated with physiotherapy, stretching and electro modalities, ayurvedic treatments will give remarkable relief from the complaints of piriformitis.
